How to become a member at Holistic Pain Management Institute: Bring your original doctor’s recommendation and your valid California ID on your first visit. Please leave yourself a few minutes on your first visit to complete our membership agreement forms. You can speed up the check in process by using the below link and completing the paperwork at home. Once our staff has verified your recommendation and paperwork you will receive a quick tour of our collective and services at which time you will be a HPMI member. Welcome!

How to check in as a returning patient: After your first visit you will have an account with HPMI, please present Cannabis ID (doctor’s reccommendation or copy or Canna card ID) as well as your vaild California ID to the front desk staff, your will be check in instantly.

Are there any age limits? You must be 18 or older.

Is there a membership fee? No membership fees. Each member contributes as much or as little as they want / need to the collective. *In order to receive special promotins, a minimum donation may be required to activate your membership.

All patients must have a valid California ID and vaild reccomendation from a lincensed California doctor.

All HPMI members agree to basic rules, best practices, and code of conduct while at HPMI and interacting with collective members.

HPMI is a peaceful place where patients have come together to help other patients with alternative medications and healing services. HPMI has been helping patients in the South Bay since early 2009.

If you have a medical cannabis ID card, you may bring that as well. Please note that medical cannabis ID cards are optional under state law. Your membership will be pending until we verify your letter of recommendation.
